Absolutely. I ordered my mandolin, an arm rest, and a Tone-Gard all at the same time. Opened the boxes, put the arm rest & Tone-Gard on the mandolin, and they've been there ever since. It even fits in the gig bag all together (although this is not always the case).

All my mandolin-family instruments have arm rests. All except my mandocello have tone-gards as well. The only reason my mandocello doesn't have a tone-gard is Tony stopped making custom gards.
As @phydaux and @Mandobart said, yes. No problem. The armrest is generally a semi-permanent thing, and goes on first, of course.
Whether all of this fits in the case is another matter. My Eastman has the Bobelock-type oblong case and it's got lots of room. The Northfield is a bit snug around the body in the Recurve case, but no vertical clearance issue. The Travelite case I have is probably going to look for a home as it's a tighter vertical fit than I really like with the Tone-Gard installed.
