Most likely made by or for Lyon & Healy as a wholesale item. Looks pretty decent.
This type of bridge would work but more than likely you will have to cut it down some for the proper height. Hopefully the neck angle gives you enough height. Placement is where you see that mark on the soundhole side of the cant (crease in top). Otherwise you can get a small piece of rosewood or ebony and carve one yourself.
Probably best to replace the tuner set with another one but you may need to find a vintage set with the same spacing, though possibly find a gear and screw that would match. Does it have a tailpiece?
String with ultra-light gauge strings. Least expensive are GHS A240 set.
