Looking carefully, I suspect that it's really a 4-on-a-plate tuner plus a cobbled-up single for that top tuner. Its plate edge doesn't quite align with the lower four, plus there's an "orphan" backplate screw wedged into a corner rather than on center & aligned w/ the tuning posts and other screws. Not that THAT helps solve the mystery!

If you squint, it looks a little like a charango, which also uses re-entrant tuning...

I posted a link over at Greif's Music Store who occasionally gets oddities. One replied: "We have NO idea. Haha. It has five strings, but not set up like a standard 5-string banjo. It looks like it has classical strings, too."

Judging by the ten (or nine) strings, and the guitar shape, I think it is a combination of a banjo and a tiple, aka a "bipple".;)
