Can you adjust the trussrod while the instrument is tuned to pitch? The neck is flat without tension.

Yes, you can, if small adjustments are all that is needed.
If the neck is flat with no string tension, there's no reliable way to know what it will do with full string tension. Most necks will be about right if they are straight without strings.
