As hamfisted as I am, I have always had trouble hanging onto even a large sized guitar pick for the duration of one song. There are special picks made with grippers and special covers, but since I am pick retention impaired, I am loathe to spend more than a few coins on a single pick.
Here's my solution. I take two standard flat picks and super glue them together with the business ends pointed at something more acute than 90°. The one pick nestles between thumb and fore finger, pointing towards the forefinger nail. This extra surface area in contact with my robust pointer creates enough friction to hold it in place better. It's kind of like why race cars use slick, wide tires. More surface contact, more friction, more stick.
The real benefit is that I don't need to keep a death grip on the pick to keep it in place. Immensely reducing right arm fatigue. http://www.mandolincafe.net/iB_html/non-cgi/emoticons/laugh.gif
I'd patent it, but I'd suspect everyone's double-pick orientation would be different. If you want to make one yourself, use double sided tape or weak glue for road testing prototypes so that you can reposition to determine your best angle.
For mine I use XH and M weight picks so I can switch as needed from song to song. I find pickin' melodies better with XH and strummin' chords better with M or H.
